Les brûlures graves représentent une urgence médicochirurgicale dont la gestion nécessite le recours à une équipe multidisciplinaire. L’évaluation initiale des lésions est primordiale car elle permet d’orienter le malade vers la structure adaptée et de définir la stratégie thérapeutique dont la mise en œuvre est urgente. En effet, la gestion des voies aériennes doit être précoce et l’indication de l’intubation parfois retenue en l’absence de détresse respiratoire. Il s’agit des brûlures du visage ou du cou associées à une exposition aux fumées d’incendie dont l’évolution peut se faire vers un œdème laryngé et des œdèmes importants du segment céphalique rendant l’intubation ultérieure difficile. L’initiation de l’expansion volémique représente une seconde urgence car de sa précocité dépend la survenue des défaillances d’organes d’évolution souvent défavorable. Cette expansion doit être précoce, rapide et intense mais doit chercher à limiter la formation d’œdèmes interstitiels pourvoyeurs de leur propre morbidité. Enfin, la troisième urgence est l’indication d’incisions de décharge nécessaires en cas de lésions profondes circulaires afin de prévenir l’ischémie d’aval. La prise en charge devra en complément intégrer un support nutritionnel adapté et un traitement local des brûlures permettant de prévenir les complications infectieuses ultérieures.
Severe burn injury represents a medico-surgical emergency involving a multi-disciplinary team. Severity assessment is essential on the scene in order to provide the most suitable care and guide the patient to the appropriate structure. Indeed, early upper airway management is mandatory and some patients need to be intubated before the occurrence of respiratory distress, especially in case of face or neck burns associated with smoke inhalation. In this situation, a delayed laryngeal oedema may occur and skin oedema may prevent a safety intubation. The initiation of volemic expansion is also an emergency to prevent hypovolemia and organ failure. The fluid infusion should be started as soon as possible, intensively but should aim at limiting the interstitial oedema formation leading to a significant morbidity. The third emergency is the evaluation of the need to perform escharrotomy in case of circumferential deep burn inducing ischemia. The standard care also implies nutritional support and wound treatment to prevent infectious complications.
