Tout medecin a une responsabilite directe dans la lutte contre la tuberculose (LAT) comportant la vaccination par la BCG et le depistage cible avec diagnostic des sujets contacts. Chez l'enfant, le diagnostic des sujets contacts est indispensable, car l'evolution est rapide de la primo-infection (PI) vers la maladie. Les deux strategies d'investigation, facilitees par la levee de l'anonymat (<< signalement >>) de la declaration obligatoire, sont : recherche de cas secondaire(s) autour d'un cas de tuberculose contagieuse nouvellement diagnostique ; recherche d'un contaminateur devant une maladie ou une PI recente de l'enfant. Le depistage se fait en cercles concentriques (des contacts etroits aux plus occasionnels). La recherche d'un contaminateur se fait par radiographie pulmonaire (RP) chez les adultes en contact etroit (famille, collectivite proche) suivi du depistage dans l'entourage du contaminateur. Autour d'un cas de tuberculose bacillifere, les PI recentes sont depistees par l'intradermoreaction a la tuberculine (IDR), completee par RP, lorsqu'elle est positive. La RP est proposee d'emblee aux personnes agees ou immunodeprimees et aux enfants de moins de 5 ans. Chez ces derniers, une RP normale n'excluant pas de petites adenopathies mediastinales, un scanner thoracique est facilement propose. L'IDR malgre les limites de sa sensibilite et de sa specificite est indispensable chez l'enfant. Toute PI de l'enfant, meme latente, doit etre traitee. En France, l'association isoniazide-rifampicine est proposee, toujours avant l'age de 3 ans, souvent jusqu'a l'age de 10 ans. La duree de 6 mois est communement admise. Les tuberculoses patentes recoivent une tritherapie ou une quadritherapie initiale de 2 mois suivie d'une bitherapie de 4 mois. Le succes du traitement repose sur la motivation et la coordination des equipes d'intervention.

The search for contaminators (family, collectivity) relies on chest radiography (CR) and if positive, is extended to the contaminator surroundings. The search for secondary cases relies on tuberculin intra-dermal test (IDR); if positive, a CR is asked for and always performed in aged or immunodepressed persons, and children under 5 years of age. For them, a thoracic scanography is easily done because of small mediastinal adenopathies undiagnosed by plain CR. Despite an unperfect sensitivity and specificity, IDR is pivotal for diagnosis in children. Any PI in children has to be treated. In France, bitherapy isoniazide-rifampicine is given before 3 years of age and often up to 10 years. A duration of 6 months is common. Patent forms receive a tri or quadri-therapy for the first 2 months followed by a 4 months bitherapy. A cornerstone of success is an adequate organisation and coordination of motivated health care workers.
