Objectif. – Évaluation de la réponse au traitement par infliximab chez des patients atteints de polyarthrite rhumatoïde (PR) associant une évaluation clinique et une évaluation des modifications tissulaires inflammatoires par imagerie par résonance magnétique (IRM).Patients et méthodes. – Nous avons inclus dans cette étude 16 patients atteints d'une PR réfractaire, traités par perfusions intraveineuses d'infliximab (3 mg/kg) aux semaines S0, S2, S6 puis toutes les huit semaines. Des examens par IRM du poignet et de la main, du côté dominant, ont été effectués avant le traitement et après 12 mois de traitement. Le volume des tissus inflammatoires (rehaussement du signal) a été évalué par des séquences en T1 avec suppression de graisse et injection de gadolinium, en utilisant le logiciel Analyze 4.0. La mesure de l'activité de la PR a été effectuée par le calcul du score du DAS28. L'amélioration clinique a été évaluée par la mesure de la réponse ACR20 (critères 20 % du Collège américain de rhumatologie).Résultats. – La population étudiée comprenait 13 femmes et trois hommes, âgés de 49,5±17,0 ans (moyenne±déviation standard), atteints d'une PR évoluant depuis 10,5±8,0 ans. Dix patients avaient des facteurs rhumatoïdes IgM. Après 12 mois de traitement par infliximab, il a été observé une diminution significative de la vitesse de sédimentation, de la CRP, du score du DAS28 et du volume des tissus inflammatoires à l'IRM. Une réponse ACR20 a été obtenue chez 14 patients (87,5 %), tandis que le taux de réponse était de 56,25 % (n=9) pour l'ACR50 et 31,25 % (n=5) pour l'ACR70. Avant traitement, il existait une corrélation positive entre le volume des tissus inflammatoires à l'IRM, et le nombre d'articulations gonflées (r=0,66), le nombre d'articulations douloureuses (r=0,79) et le score du DAS 28 (r=0,57).Conclusions. – Chez les patients atteints d'une PR réfractaire, l'adjonction d'un traitement par infliximab entraîne une amélioration des paramètres cliniques et biologiques, ainsi que des anomalies synoviales à l'IRM. L'évaluation par IRM du volume des tissus inflammatoires se rehaussant après injection de gadolinium, pourrait constituer une méthode d'analyse supplémentaire de l'activité synoviale de la PR et de sa réponse au traitement.
Aim. – To investigate the clinical response and to evaluate by magnetic resonance imaging the inflammatory tissue changes in refractory rheumatoid arthritis patients treated with infliximab.Methods. – Sixteen refractory rheumatoid arthritis patients who were treated with intravenous infliximab (3 mg/kg) at weeks 0, 2, 6 and every eight weeks thereafter, were examined with magnetic resonance imaging of the dominant affected wrist and hand before treatment and one year after therapy. The volume of the enhancing inflammatory tissue was evaluated in fat suppressed contrast enhanced T1–weighted images by using the Analyse 4.0 software. Disease activity was evaluated by assessing the disease activity score for 28 joint indices. The clinical improvement was evaluated according to the American College of Rheumatology 20% response criteria.Results. – There were 13 females and three males with mean age 49.5 (17.0) years and mean disease duration 10.5 (8.0) years. Ten patients had positive IgM rheumatoid factor. One year after treatment, a significant reduction of the erythrocyte sedimentation rate, the C-reactive protein, the disease activity scores for 28 joint indices and the volume of the enhancing inflammatory tissue were observed. All but two of the rheumatoid arthritis patients achieved the American College of Rheumatology 20% response criteria, while nine (56.25%) and five (31.25%) patients achieved the 50 and 70% American College of Rheumatology response criteria respectively. A positive correlation between the volume of the enhancing inflammatory tissue, swollen joint count, tender joint count, as well as disease activity score for 28 joint indices (R=0.66, R=0.79, R=0.57 respectively) was found before treatment.Conclusions. – In refractory rheumatoid arthritis patients, the addition of infliximab therapy may result in clinical, laboratory and magnetic resonance imaging improvement. Magnetic Resonance Imaging assessment of the volume of the enhancing inflammatory tissue may represent an additional tool for the investigation of joint disease activity and responsiveness to treatment.
