In unserer Klinik wird seit über 30 Jahren die operative Behandlung von Klumpfüßen über einen dorsomedialen Zugang durchgeführt. Postoperativ erfolgt zunächst eine Gipsbehandlung, dann eine konsequente Nachbehandlung mit Orthesen und Winkeleinlagen. Zwischen dem 1. 6. 1986 und dem 31. 12. 2000 wurden 130 primäre Klumpfußweichteiloperationen bei 86 Patienten durchgeführt. 119 (92%) Füße konnten nach durchschnittlich 4,5 Jahren klinisch-radiologisch nachuntersucht werden. Das durchschnittliche Alter bei der Operation betrug 7,6 Monate. Klassifikation nach Dimeglio und Score nach Laaveg und Ponseti dienten zur Beurteilung. Die gemessenen Winkel in den belasteten Rönt-genbildern des Fußes a.p. und seitlich prä- und postoperativ wurden verglichen, Komplikationen und Rezidive dokumentiert. 29 (22,3%) Füße wiesen einen Schweregrad IV nach Dimeglio, 58 (44,6%) Grad III und 43 (33,1%) Grad II auf. Ein Klumpfußrezidiv war bei 9 (7,6%) Fällen zu beobachten. Alle Winkel verbesserten sich signifikant. Bei der Nachuntersuchung wurden durchschnittlich 95,4 ± 9,2 Punkte nach dem Score von Laaveg und Ponseti erzielt. Der von uns verwendete dorsomediale Zugangsweg erlaubt eine ausgezeichnete Klumpfußkorrektur mit geringer Rezidiv- und Komplikationsrate.
In our clinic for more than 30 years we have used a dorsomedial approach in the operative treatment of clubfeet. Postoperative a plaster dressing is included, followed by a consequent therapy with orthosis and supports. Between 01 June 1986 and 31 December 2000, 130 primary clubfoot operations with soft tissue release in 86 patients were performed. 119 (92%) feet could be clinically and radiologically followed-up after a mean of 4.5 years. Average age at time of operation was 7.6 months. Classification according to Dimeglio and scoring according to Laaveg and Ponseti were used for evaluation. X-rays with load (a.p. and lateral view) were evaluated pre-operatively and at follow-up; complications and recurrences were documented. 29 (22.3%) of the feet were classified IV°, 58 (44.6%) III° and 43 (33.1%) II° according to Dimeglio. Recurrent clubfoot was found 9 (7.6%) times. All angles improved significantly. At latest follow-up 95.4 ± 9.2 points according to the score of Laaveg and Ponseti were achieved. The dorsomedial approach allows an excellent correction of clubfeet with a low rate of complications and recurrences.
