L’arrêté du 20/9/2000 modifiant le titre premier du tarif interministériel des prestations sanitaires et relatif aux matériels et nutriments nécessaires à la nutrition entérale à domicile a défini des prestations techniques, administratives et générales réalisées par des prestataires de santé à domicile. Le but de cette étude a été de réaliser une analyse descriptive de la population prise en charge par nutrition entérale à domicile depuis la parution de l’arrêté et d’en déterminer les tendances évolutives.Il s’agit d’une étude observationnelle rétrospective réalisée à partir de données épidémiologiques recueillies prospectivement sur l’ensemble des malades pris en charge par nutrition entérale à domicile par l’entreprise Nestlé Home Care entre 10/2000 et 01/2015. Une analyse de l’évolution des données a été réalisée en comparant trois périodes P1 : 10/00 à 7/05 ; P2 : 08/05 à 04/10 et P3 : 05/10 à 01/15. Les analyses statistiques ont été réalisées par Anova et des tests paramétriques de comparaison de proportions (méthode Monte Carlo et procédure de Marascuilo). Une différence était significative* pour p<0,05.En 15ans, 39 275 patients (H : 65 %, F : 35 %) d’âge moyen 55±23ans ont été pris en charge (perte de poids 14±21 % ; IMC : 21,7±6,4). Les principaux établissements d’origine étaient un CHU (37,8 %) un CH (29,6 %), une clinique (12,5 %) et un CLCC (5,7 %). Les patients étaient répartis dans tous les départements de France sans corrélation entre la prévalence de la nutrition entérale à domicile et la population du département (rho=0,073). Il y avait 11 % d’enfants, 56 % d’adultes et 30 % de sujets âgés de plus de 70ans dont la prévalence diminuait (P1=33,4 % ; P2=29,8 % ; P3=27,1 %) au profit des adultes. La durée moyenne de prise en charge était de 259±402 j et diminuait (P1=380±671 j, P2=259±402 j, P3=132±188 j). Les principales pathologies étaient les cancers ORL (34,9 % : P1=37,4 %, P2=32 %*, P3=35,5 %), les affections neurologiques (13,4 % : P1=19,2 %, P2=14,9 %*, P3=10,6 %*), la maladie de Crohn (11,2 % : P1=9,7 %, P2=8,1 %, P3=13,1 %*), les affections digestives diverses (8,4 % : P1 9,4 %, P2=7,1 %, P3=8,9 %), le polyhandicap (10,5 % : P1=11 % ; P2=15,7 %*, P3=7,8 %*), les cancers (7,7 % : P1=1,8 %, P2=6,8 %*, P3=10,4 %*), les troubles du comportement alimentaire (2,7 % : P1=1,6 %, P2=4,0 % P3=2,4 %), la maladie d’Alzheimer (1,3 % P1=2,4 %, P2=1,7 %, P3=0,6 %*) et des causes diverses. La nutrition entérale à domicile a été réalisée à l’aide d’une pompe dans 79 % des cas et son utilisation augmentait (P1=72,6 %, P2=80,6 %*, P3=79 %*). Soixante et un pour cent des patients avaient une gastrostomie dont l’utilisation diminuait au profit des sondes naso-gastriques (33 % : P1=24,9 %, P2=28,6 %, P3=40,6 %*) et 6 % une jéjunostomie. Vingt-sept pour cent des patients géraient eux-mêmes la nutrition entérale à domicile ; 62,6 % recevaient un mélange polymérique sans fibre dont l’utilisation augmentait (P1=48,9 %, P2=62 %*, P3=62,6 %*) au profit des mélanges avec fibres (29,4 % ; P1=42,4 %, P2=30,7 %, P3=29,4 %) et 8 % un produit semi-élémentaire. Quarante-trois pour cent des patients recevaient une nutrition entérale à domicile exclusive. Les apports énergétiques et protéiques initiaux moyens étaient respectivement de 1529±704kcal/j et 67,5±31g/j.Cette cohorte qui représente environ 25 % des malades pris en charge en France, est un bon reflet de la pratique de la nutrition entérale à domicile dans notre pays. Elle permet de dégager d’importantes tendances évolutives.
