L’asthme de l’enfant en milieu équatorial a été peu étudié. Les pathologies respiratoires sifflantes sont fréquentes chez les enfants consultant à l’hôpital Albert Schweitzer. L’objectif de cette étude a été de préciser le diagnostic d’asthme en y associant un bilan allergologique et fonctionnel.Soixante-treize patients âgés de 2 à 18ans ont été inclus de manière consécutive de janvier à juillet 2014. Un interrogatoire à l’aide d’un questionnaire standardisé précisait la gravité de l’asthme, les signes associés (rhinite, eczéma) ainsi que les facteurs environnementaux. La mesure du VEMS pré- et post-bronchodilatateur ainsi que des prick-tests vis-à-vis des acariens Dp et Df, des allergènes de chat, chien, blattes, graminées, moisissures et d’arachide ont été réalisés.Parmi les 73 patients, 23 avaient un âge≤5ans. On notait un ratio fille/garçon de 30/43. Six patients présentaient plutôt une toux spasmodique, 2 une bronchiolite et dans 8 cas, le diagnostic d’asthme était incertain. Des signes cliniques évoquant une dermatite atopique ou une rhinite ont été signalés respectivement dans 22 et 51 cas. Chez 57 enfants où le diagnostic d’asthme a été retenu, les scores de gravité ont montré un pourcentage peu élevé d’asthme persistant sévère (2 cas), 3 cas d’asthme persistant modéré, 15 cas d’asthme persistant léger et 37 cas d’asthme intermittent. Sur 47 cas d’asthme où l’exploration fonctionnelle était réalisable, seuls 4 enfants avaient un syndrome obstructif sévère (VEMS<60 %) et 24 avaient un VEMS entre 60 et 80 %. Sur les 72 ayant eu un bilan allergologique, la majorité était sensibilisée aux acariens (46 %), 34 % aux blattes, 19 % au chat, 18 % au chien, 18 % aux graminées, 7 % à Alternaria. Notons que 15 % des enfants testés étaient sensibilisés à l’arachide, dont 3 avaient une allergie cliniquement prouvée.L’existence d’asthme dans la population pédiatrique en milieu semi-rural au Gabon est confirmée par cette étude. Elle met en exergue la faible fréquence des asthmes graves. Le bilan allergologique a identifié dans 2/3 des cas des sensibilisations aux pneumallergènes de l’environnement domestique, ce qui devrait conduire à une meilleure prise en charge globale de ces patients.
