Le but de l’étude est de montrer l’impact thérapeutique de la scintigraphie rénale au 99mTc-DMSA dans la mutité rénale à l’urographie intraveineuse (UIV).Il s’agit d’une étude rétrospective monocentrique, colligeant 10 patients en mutité rénale à l’urographie intraveineuse (UIV) pour lesquels une scintigraphie rénale au 99mTc-DMSA a été indiquée. Une scintigraphie rénale au 99mTc-DMSA a été réalisée chez tous nos patients au moyen d’une gamma-caméra à grand champ, munie d’un collimateur parallèle de faible énergie et à haute résolution. L’évaluation des images obtenues a consisté en une étude qualitative du parenchyme ainsi qu’une estimation de la valeur fonctionnelle séparée des deux reins, obtenue par le calcul de la moyenne géométrique.La moyenne d’âge était de 17 ans avec des extrêmes allant de 16 mois à 76 ans. La mutité rénale étiquetée à l’UIV était due chez 4 patients à une origine lithiasique, chez 4 patients à un syndrome de jonction pyélo-urétérale et chez 2 patients à une pyélonéphrite chronique. La fonction rénale relative des reins étiquetés muets à l’UIV, obtenue grâce à la scintigraphie rénale au 99mTc-DMSA, variait de 5 % à 20 % avec une moyenne de 12 %.La scintigraphie rénale au 99mTc-DMSA a permis, à travers notre série, de palier aux limites de l’UIV quant à l’évaluation précise de la valeur fonctionnelle des reins pathologiques. En cas de rein muet, le recours à la scintigraphie reste indispensable pour une décision thérapeutique adéquate, puisque 55 % des reins muets à l’UIV ont une FRR>5 %.
