The aim of this study was to determine community application of cardiopulmonary resuscitation (CPR) skills in an emergency, and, thus, assess the value of training programmes in raising community competence. A cross-sectional telephone survey of the Western Australian population was chosen randomly (n=803). An urban sub-sample (n=100) performed a practical demonstration of CPR skills using a simulated collapse scenario using a recording manikin as the victim. Performance was assessed by two observers using pre-determined criteria. Of all respondents, 64% had been trained in CPR. Practical and theoretical assessment scores were significantly better in trained versus untrained participants. The number of times a person was trained in CPR was more effective for retention and competence than time since last trained. Degree of training and theoretical competence were less in those aged over 65 years or with heart disease in the household. Theoretical competence poorly reflected practical performance in many tasks. This study provides a comprehensive database of CPR training and performance, and highlights future directions to ensure appropriate and cost-effective training. Specific factors to be addressed include increasing frequency of training, targeting of high-risk groups, simplification in teaching, and emphasising early activation of the emergency medical system.
O objectivo deste estudo foi determinar a capacidade para realizar reanimação cardio-pulmonar (RCP) na comunidade, em caso de emergência, e portanto avaliar a capacidade de um programa de treino em elevar a competência da comunidade nesta área. Foi realizado um questionário por via telefónica à população de Western Austrália (n=803), cruzado de forma randomizada. Fez-se com uma sub-amostra (n=100) uma demonstração prática de competência em CPR usando um cenário simulado de colapso com um manequim com possibilidade de registo. O desempenho foi avaliado por dois observadores usando critérios pré-determinados. 64% de todos os que responderam foram treinados em RCP. Os scores de avaliação prática e teórica foram significativamente melhores nos participantes treinados do que nos não treinados. O grau de treino e a competência teórica foram menores naqueles com idade superior a 65 anos ou com doença cardı́aca. A competência teórica reflecte mal o desempenho prático em muitas tarefas. Este estudo fornece uma base de dados compreensiva do treino e do desempenho RCP, e assinala direcções futuras para assegurar treino apropriado e a baixo custo. Factores especı́ficos a assinalar incluem aumento da frequência de treino, direccionar a formação para grupos de alto risco, simplificar o ensino e enfatizar a activação precoce do sistema de emergência médica.
El objetivo de este estudio fue determinar la aplicación de las destrezas de (RCP) reanimación cardiopulmonar en una emergencia, y, ası́, medir el valor de los programas de entrenamiento en mejorar la competencia de la comunidad. Una encuesta telefónica, seccional cruzada, de la población de Australia oeste elegida randomizadamente (n=803). Una sub muestra (n=100) realizó una demostración práctica de destrezas de RCP usando un escenario simulado de colapso usando un maniquı́ con sistema de registro. El desempeño fue evaluado por dos observadores usando criterios predeterminados. De todos los que respondieron, 64% habı́a sido entrenado en RCP. Los puntajes de evaluación práctica y teórica fueron significativamente mejores en participantes entrenados que en los no entrenados. El número de veces que fue entrenada una persona en RCP fue más efectivo para la retención y competencia que el tiempo transcurrido desde el último entrenamiento. El grado de entrenamiento y competencia teórica fue menor en aquellos de más de 65 años de edad o con enfermedad cardiaca en su ambiente familiar. En muchas destrezas, la competencia teórica malamente refleja el desempeño práctico. Este estudio proporciona una base de datos de entrenamiento en RCP y desempeño, y destaca direcciones futuras para asegurar entrenamiento apropiado y costo- efecto. Los factores especı́ficos que deben abordarse incluyen frecuencia creciente de entrenamiento, identificación de grupos de alto riesgo, simplificación en la educación, y enfatizar la activación temprana del sistema de emergencias médicas.
