La spondylarthrite ankylosante (SPA) est fortement associée à HLA-B27. Cependant, la force d’association entre HLA-B27 et les paramètres cliniques peut varier en fonction de la situation géographique. Le but de cette étude était de comparer les paramètres cliniques de la SPA et la fréquence de HLA-B27 et ses allèles chez des patients turcs et des patients d’autres séries.Cent-douze patients (72 hommes et 40 femmes) remplissant les critères de classification modifiés de New York pour la SPA et 55 (27 hommes et 28 femmes) sujets sains étaient typés pour HLA-B27 et ses allèles par des oligonucléotides spécifiques (PCR/SSP). Une évaluation clinique était de plus réalisée.Le rapport homme/femme était de 1,8, l’âge moyen d’apparition de la maladie de 23,5 ans et 24,1 % des patients avaient développé les premiers signes dans leur jeunesse. Des atteintes articulaires périphériques étaient mises en évidence chez 52,7 % des patients. Les antécédents familiaux (p=0,01) et les arthrites périphériques (p=0,02) étaient plus fréquents chez les femmes alors que c’était plutôt le rachis qui était atteint chez les hommes. Soixante-dix pour cent des patients étaient positifs pour HLA-B27 et associé à un âge moyen moins élevé, une uvéite et à un temps réduit entre les premiers symptômes et le diagnostic. La fréquence des allèles de HLA-B27 associés à la SPA n’était pas différente entre les patients atteints de SPA et les sujets sains.La faible fréquence de HLA-B27 et les variations cliniques de la SPA pourraient être dus à d’autres facteurs génétiques ou environnementaux en Turquie.
