Case report: To report a rare case of optic neuropathy in refractory neurosarcoidosis treated with tumour necrosis factor-α antagonist (infliximab) perfusion. A 41-year-old man with diabetes presented with an abrupt, painless, unilateral visual loss. Thoracic computed tomography disclosed mediastinal lympha-denopathy, and evidence from histopathology confirmed sarcoidosis.Comments: Corticosteroids alone and with immunosuppressive agents failed to suppress the active disease. Alternative therapy of infliximab perfusions resulted in disease remission and relative visual improvement, and this treatment may warrant further clinical studies.
Observation: Compte rendu d’un rare cas de neuropathie optique dans le cadre d’une neurosarcoïdose réfractaire, traité par perfusion d’un antagoniste du facteur-α nécrosant des tumeurs (infliximab). Un homme diabétique de 41 ans s’est présenté avec une perte visuelle unilatérale soudaine et indolore. L’examen de tomodensitométrie thoracique a révélé une lymphadénopathie médiastinale, et des résultats hystopathologiques ont confirmé la sarcoïdose.Commentaires: Les corticostéroïdes, seuls ou avec des agents immunodépresseurs, n’ont pas réussi à éliminer la maladie active. L’autre traitement par perfusion d’infliximab a produit une rémission et une relative amélioration visuelle; ce traitement peut justifier des études cliniques.
