L’émergence des résistances en infectiologie rend la thérapeutique de plus en plus difficile. Les mécanismes génétiques et biochimiques, responsables de la résistance des champignons aux antifongiques permettent de mieux comprendre l’épidémiologie de cette résistance et de mieux appréhender les facteurs responsables de la sélection des souches multirésistantes. L’accumulation de mutation, l’amplification de loci conduisent à amplifier les phénomènes de multirésistance. D’un autre côté, l’implication des pompes à efflux dans la multirésistance en mycologie médicale occupe une place très considérable. Les pompes à efflux ont été trouvées aussi bien chez les procaryotes que les eucaryotes. Malgré la caractérisation d’un nombre croissant de pompes et le développement de nouvelles classes d’antifongiques, les conséquences cliniques de la résistance par efflux sont très inquiétantes. Dans cette revue générale, nous décrivons les différentes classes de transporteurs et particulièrement les transporteurs ABC et leurs implications en infectiologie. Face à l’émergence des souches multirésistantes, il est nécessaire de trouver de nouvelles molécules, actives contre les levures et les champignons pathogènes ou qui inhibent leurs mécanismes de résistance et restaurent ainsi l’activité des antifongiques existantes. L’utilisation d’inhibiteurs spécifiques des transporteurs ABC semble constituer une approche thérapeutique intéressante. Cette revue souligne les principaux inhibiteurs des transporteurs de pompes à efflux décrits, leurs origines et leurs mécanismes d’action.
The emergence of resistance makes therapy in infectious diseases more difficult. The origin of the mounted resistance is probably multifactorial. The genetic and biochemical mechanisms responsible for yeast resistance to antifungal drugs used to better understand the epidemiology of the spread and selection of resistance strains. The accumulation of mutations and the amplification of loci lead to the amplification of resistance phenomena. On the other side, the involvement of efflux pumps in multidrugs resistance is large in medical mycology. Efflux pumps have been found both in prokaryotes and eukaryotes. Even with the characterization of a growing number of pumps and the development of new classes of antifungal drugs, the resistance by efflux still very serious. This review describes the different classes of efflux pumps, particulary the ABC transporters and their involvement in infectious diseases. In view of the emergence of multi-resistance strains to antifungal drugs, it is necessary to find new active molecules against yeast and fungi, or inhibit their resistance mechanisms and thus restore the activity of existing drugs. The use of specific inhibitors of pumps appears to be an interesting therapeutic approach. This review highlights the main ABC transporters inhibitors, their origins and their mechanisms of action.
