About 1–3% of women experience early recurrent miscarriages, defined by ≥3 fetal loss before 14 weeks of gestation. About half of these recurrent early miscarriages could be related to a genetic cause. Pre-implantation genetic diagnosis is used in several European countries, but it is still prohibited in France except for couples at risk for transmission of severe genetic diseases. The immune dysregulation, and in particular allo-immune excessive response, could be responsible for fetal loss in remaining cases, although currently we lack biomarker to confirm the immune-mediated fetal loss. Several immunosuppressive and immunomodulatory treatments have been the subject of small studies in patients with early recurrent miscarriages. The available data do not allow to define the treatment recommendations in this topic, and further studies are necessary.
Environ 1 à 3 % des femmes en âge de procréer présentent des fausses couches précoces récidivantes, définies par ≥3 pertes fœtales avant 14 semaines de gestation. Environ la moitié de ces fausses couches précoces inexpliquées pourraient être en relation avec une cause génétique. Bien que le dépistage préimplantatoire soit utilisé en médecine de la reproduction dans de nombreux pays, cette technique est interdite en France en dehors du cadre précis du « diagnostic préimplantatoire », concernant les couples à risque de transmission de maladie génétique grave. Dans les autres cas, une réponse allo-immune du fait d’un défaut d’induction de tolérance vis-à-vis des antigènes paternels pourrait être à l’origine de la perte fœtale, bien que l’on manque actuellement de marqueur disponible en routine pour affirmer l’origine immunitaire de la perte fœtale. Plusieurs traitements immunomodulateurs ont fait l’objet d’études ouvertes ou randomisées chez les patientes présentant des fausses couches précoces inexpliquées, parmi lesquels les corticoïdes, le G-CSF, les immunoglobulines, les intralipides et les antagonistes du TNF-α. Les données disponibles ne permettent pas actuellement de proposer des recommandations, mais des études sont nécessaires pour déterminer des stratégies pour ces patientes.
