Faire le point sur la technique du ganglion sentinelle dans la prise en charge du cancer du sein à l’institut Salah Azaiez de Tunis, plus de dix ans après la validation de courbe d’apprentissage.Il s’agit d’une étude rétrospective entre 2013 et 2015 colligeant 100 patientes atteintes de tumeur maligne du sein (taille<2cm sans adénopathies cliniquement palpable). La moyenne d’âge était de 45,12 ans. Ces patientes ont toutes bénéficié d’une lymphoscintigraphie isotopique préopératoire réalisée au service de médecine nucléaire la veille de l’opération, elle consistait à l’injection d’une activité moyenne de 1,2 mCi de nanocolloïdes marqués au 99mTc en péri-aréolaire, suivie d’une acquisition statique 90minutes plus tard, couplée à la méthode colorimétrique en peropératoire.Dans tous les cas, la lymphoscintigraphie préopératoire s’est déroulée au service de médecine nucléaire. Nous avons détecté 114 ganglions chauds. La méthode colorimétrique en peropératoire a détecté 100 ganglions bleus. L’étude histologique des ganglions chauds et bleus réséqués a trouvé 17 cas de ganglions envahis aussi bien à l’examen extemporané que définitif, et 79 cas de ganglions sains aussi bien à l’examen extemporané que définitif.La procédure ganglion sentinelle est la technique recommandée pour l’exploration chirurgicale axillaire dans le cancer du sein localisé sans atteinte clinique ou radiologique ganglionnaire axillaire.
