La médiastinite descendante nécrosante qui peut compliquer une infection dentaire ou ORL négligée est une affection exceptionnelle et grave d'évolution souvent mortelle au stade des complications pleuromédiastinales. Le diagnostic clinique repose sur l'existence d'un trismus, d'une dyspnée et surtout d'une crépitation dans la région cervicothoracique en rapport avec un emphysème souscutané. La tomodensitométrie cervicothoracique confirme le diagnostic au stade de début et permet d'orienter les drainages et de suivre l'évolution. Le traitement repose sur un drainage large cervical et/ou thoracique en fonction de l'étendue de la cellulite, associé à une antibiothérapie à large spectre et à une réanimation adaptée. L'évolution a été favorable dans cette observation.
Descending necrotizing mediastinitis which may occur as a complication of neglected oropharynx infection is an uncommon disease, although it is lethal in most cases. Trismus and dyspnea are usual with palpable crepitation located in the cervico-thoracic area. Early diagnosis can be confirmed with accuracy by CT scan. Treatment is based on early mediastinal drainage by cervical approach, intravenous antibiotics and reanimation. The reported case had a favorable outcome.
