Les techniques de cœlioscopie ainsi que l’instrumentation se sont développées ces dernières années et permettent actuellement de réaliser de nombreuses interventions. La splénectomie chez l’enfant est essentiellement indiquée en cas d’hémopathie. Patients et méthodes. – Nous rapportons notre expérience de 23 cas (âgés de trois à 14 ans) de splénectomie sous laparoscopie (sphérocytose héréditaire 〚neuf cas〛, drépanocytose 〚quatre〛, purpura thrombopénique idiopathique 〚trois〛, autres causes 〚trois〛). La technique est décrite. Dans trois cas, une cholécystectomie était associée. Un seul cas a dû être converti. Résultats. – La durée d’intervention était en moyenne de 170 min (extrêmes : 115–230 min). La sortie était possible en moyenne au sixième jour (extrêmes : 3–15 j). Trois enfants ont eu des complications respiratoires et deux un abcès profond, traité par antibiothérapie. Le recul varie de deux mois à quatre ans sans aucune complication en rapport avec l’intervention. Conclusion. – La splénectomie sous cœlioscopie est devenue une technique de choix chez l’enfant. Elle a supplanté dans notre service les techniques à ciel ouvert.
Pediatric laparoscopic splenectomy is a relatively new surgical procedure. Advances in instrumentation and technique now make this procedure possible. Splenectomy is frequently performed in children for various hematologic and autoimmune diseases. Patients and methods. – This article reviews the indication for splenectomy, the technique of laparoscopic splenectomy and our results. Between January 1996 and January 2000, 23 children underwent laparoscopic splenectomy. Three of them also had a concomitant cholecystectomy. Their ages ranged from three to 14 years. Nine children had hereditary spherocytosis, four were affected by sickle cell disease, three had an idiopathic thrombocytopenia and three a hemolytic disease. One patient was converted. Results. – Mean operative time was 170 min (range, 115–230 min). Hospital stay ranged from three to 15 days (median, 6). Five patients had complications (three pneumonia and two deep abcesses). Median follow-up was 20 months (two months to four years) without problems regarding procedure. Conclusion. – Regarding the low complication rate for this type of patient and the advantages of a small abdominal trauma in the postoperative period, the laparoscopic approach for elective splenectomy in hematological disorders is the technique of choice.
