La maladie veineuse thromboembolique (MVTE) est une pathologie fréquente dont le risque de récidive est de 25 % à 5ans et de 30 % à 10ans, quelle que soit la localisation. Le but de notre travail était d’étudier les particularités épidémiologiques, cliniques des thromboses veineuses profondes (TVP) récidivantes des membres inférieurs (MI) et d’analyser les facteurs de risque afin de déduire le profil des patients à haut risque de récidive.Étude rétrospective incluant 32 patients colligés sur une période de 21ans allant de 1995 à 2016 pour une TVP récidivante, parmi 328 hospitalisés dans un service de médecine interne pour une TVP des MI. Le caractère récidivant a été défini sur la présence d’au moins deux épisodes de TVP des MI avec un premier épisode correctement traité.La TVP des MI était récidivante dans 10,25 % des cas. Il s’agissait de 17 hommes et de 15 femmes (sex-ratio : 1,13). Le nombre moyen de récidive était de1,78±1,1 [1–5]. L’âge moyen de nos patients au moment du premier épisode était de 38,9ans. Le délai moyen entre les deux premiers épisodes était de 8,65 années±7,31 (1–28ans). Le caractère idiopathique du premier épisode de TVP était retrouvé dans 57,14 % des cas. Les facteurs de risque (FDR) permanents de TVP identifiés chez nos patients étaient : les antécédents personnels de MVTE (100 %) ou familiaux (18 %), la thrombophilie constitutionnelle (25 %) répartis en résistance à la protéine C activée (RPCA) (4 cas), déficit en protéine C (2 cas), déficit en protéine S (1 cas) et déficit en antithrombine (1 cas). Un syndrome des anti-phospholipides (SAPL) secondaire à un lupus érythémateux systémique (LES) (3,1 %), une maladie de Behçet (9,4 %) et un cancer solide (13,1 %). Pour les FDR transitoires, il s’agissait de : varices des membres inférieurs (68,8 %), obésité (37,5 %), immobilisation récente (15,6 %), sepsis (15,6 %), surcharge pondérale (12,5 %), œstroprogestatifs (6,3 %). La grossesse, l’érysipèle et la polyglobulie étaient observés dans respectivement 3,1 % des cas. Une hyperhomocystéinémie a été observée dans 21,9 % des cas. Le nombre moyen de FDR était de1,39±0,988 (0–4).Dans notre étude comme dans la littérature, le caractère idiopathique du premier épisode de TVP a été le plus fréquemment associé au risque de récidive. La thrombophilie constitutionnelle a été relativement fréquente dans notre étude par rapport à la littérature d’où l’intérêt de la rechercher systématiquement. Alors que les cancers solides, le SAPL étaient plus rare probablement à cause du faible effectif de notre étude.
