Trois syndromes doivent être recherchés dans l’étiologie d’une douleur du gril costal antérieur : le syndrome de Tietze, le syndrome de Cyriax et la xiphodynie. Ils peuvent être à l’origine de douleurs trompeuses mimant une affection thoracique ou digestive. Le syndrome de Tietze est en rapport avec une inflammation du cartilage costo-sternal, en général de la 2 e ou 3 e côte. Le syndrome de Cyriax consiste en des épisodes répétés de luxation de l’extrémité antérieure d’une fausse côte en arrière et en haut, avec une compression du nerf inter-costal correspondant. La xiphodynie désigne une douleur de l’appendice xiphoïde, liée dans certains cas à sa saillie vers l’avant, sous la peau. Le traitement de ces trois syndromes est avant tout conservateur (infiltrations) et exceptionnellement chirurgical.
Three syndromes should be looked after, when considering a pain of the anterior chest wall: the Tietze syndrome, the painful rib syndrome and xiphodynia. The pain can be misleading, mimicking thoracic or abdominal pain. The Tietze syndrome is related to an inflammation of the costo-sternal cartilage, mainly the 2nd or the 3rd. The painful rib syndrome consists of a repeated abnormal and painful movement of the tip of a false rib backward and upward, with an impingement of the corresponding intercostal nerve. Xiphodynia corresponds to a pain from the xiphoid process, related in certain cases to its forward protrusion under the skin. The treatment of these three syndromes is mainly conservative (injections) and very rarely surgical.
