Introduction.<space>- L'empyeme pleural primitif ou secondaire a une pneumopathie infectieuse reste a l'origine d'une surmortalite et d'hospitalisations prolongees. L'age moyen de survenue d'un empyeme pleural est habituellement de 55<space>ans chez l'adulte et demeure rarement diagnostique chez le sujet tres age (85<space>ans et plus). Nous rapportons ici deux observations d'empyeme pleural survenu chez deux patients tres ages illustrant la diversite des presentations cliniques.Exegese.<space>- Les facteurs de risque d'empyeme pleural sont lies aux etats d'immunodepression habituellement decrits pour les syndromes septiques. Les germes responsables sont les cocci Gram positifs, les germes anaerobies et de facon moins frequente les bacilles Gram negatifs. Il existe toutefois une proportion elevee d'infections polymicrobiennes. L'histoire clinique peut cependant etre heterogene sans element favorable a l'origine infectieuse de l'epanchement pleural. La rapidite de la prise en charge therapeutique associant antibiotherapie et drainage pleural est un des facteurs pronostics les plus importants.Conclusion.<space>- En raison de la diversite la presentation clinique des empyemes chez le sujet age et du benefice de leurs traitements precoces, tout epanchement pleural non connu doit etre investigue.
Introduction.<space>- Primitive thoracic empyema or empyema secondary to pneumonia represents a significant medical problem necessitating prolonged hospitalizations and increasing the rates of morbidity and mortality. Average patient age is 55 years and remains rare in very old patients (><space>85). We report here two observations in very old patients, pointing out the diversity of the clinical presentation.Exegesis.<space>- Empyema thoracis is classically associated with underlying diseases. It is often polymicrobial but Gram-positive bacteria and anaerobes are more frequent than Gram-negative bacteria. The clinical presentation may be aspecific without any parameters in favor of infectious disease: the rapidity of pleural drainage and antibiotic therapy are the main prognostic factors.Conclusion.<space>- Because of the diversity of clinical presentation in the elderly population, and because of the efficiency of rapid treatment, geriatric physicians should perform an examination of pleural fluid when pleural effusion is unknown.
