La rupture de l’estomac est une complication rare des réanimations cardiopulmonaires et de l’oxygénothérapie nasale. Nous rapportons le cas d’un pneumopéritoine survenu au décours d’une séance de ventilation non invasive, dans le cadre d’un sevrage respiratoire en postopératoire d’une résection antérieure du rectum. Le pneumopéritoine a été confirmé par une radiographie centrée sur les coupoles diaphragmatiques. Son apparition a été précédée de vomissements, de troubles de la conscience puis d’un emphysème sous-cutané géant. La prise en charge chirurgicale objectivait une rupture gastrique en plusieurs points. La patiente bénéficiait d’une suture gastrique simple réalisée par laparotomie. L’évolution a été rapidement favorable.
Noninvasive positive-pressure ventilation (NIPPV) is a safe method to treat acute respiratory failure and is known to decrease the need for intubation and the length of ICU-stay. Few severe complications have been reported even when the indications are respected. Some rare cases of gastric distension were recently described. We report the case of a gastric perforation associated with NIPPV. The treatment was closure with a primary interrupted two-layer suture. Recovery was complete and the patient was transferred to the ward on Day 11.
