PKAD est la plus fréquente des maladies rénales héréditaires responsable de l’insuffisance rénale chronique terminale (IRCT). Son diagnostic est facile et repose sur l’échographie. Le but de notre travail est d’évaluer le profil clinique, épidémiologique et évolutif de la PKAD ainsi que les différents facteurs de progression de cette maladie.Étude rétrospective incluant 283 cas de PKR suivis dans le service (2000–2013) dont la durée de suivi est supérieure à une année. Nous avons déterminé les circonstances de découverte, le profil évolutif de la fonction rénale (FR) et les facteurs pronostiques associés à la dégradation de la FR. Les données cliniques et biologiques sont recueillies à partir des dossiers médicaux des patients. L’étude génétique n’a pas été réalisée.L’âge moyen des patients est de 48,81±16,62ans, 142 hommes et 141 femmes. Les circonstances de découverte sont l’insuffisance rénale (IR), les lombalgies, l’HU macroscopique, l’infection urinaire (IU), la lithiase rénale, le dépistage familial et la découverte fortuite dans respectivement 22,6 %, 17,3 %, 3,9 %, 3,2 %, 2,9 %, 4,6 % et 45,6 % des cas. L’HTA est notée chez 59,4 % patients, et l’obésité dans 12,72 % des cas. La médiane de la créatininémie au moment du diagnostic est de 12 [9–5] m/L, ce qui correspond à une clairance selon MDRD de 31 [16–74]mL/min. Après une moyenne de suivi de 24±7 mois, 18 % sont en IRCT, 17 % ont aggravé leur fonction rénale, 19 % ont gardé une IR stable et 34 % ont gardé une FR normale. Les perdus de vue sont de 12 %. Les facteurs de risque étudiés (l’âge, le sexe, l’obésité, l’infection urinaire, l’HU macroscopique, l’HTA, le taux d’Hb et de la PU de 24h)Seules l’HTA et la protéinurie qui sont associées à la dégradation de la FR au cours de la PKAD en analyse uni- et multivariée (respectivement p=0,044 et p=0,046).L’évolution insidieuse et progressive de la PKAD vers l’IR terminale constitue la gravité de la maladie. De nombreux facteurs pronostiques de progression de l’IR sont décrits dans la littérature et dont certains sont soulevés dans notre série : génétiques (PKD1, PKD2), cliniques (l’âge avancé, l’HTA, lithiases rénales) et biologiques (l’anémie et l’albuminurie).
