Une anticoagulation est le plus souvent nécessaire au cours de l’épuration extrarénale continue pour une utilisation optimale du filtre. L’héparine non fractionnée demeure l’anticoagulant de référence. Elle présente néanmoins quelques inconvénients comme une augmentation du risque hémorragique, une résistance à l’héparine ou encore une thrombopénie induite par l’héparine. Des molécules antithrombotiques alternatives sont disponibles, avec leurs propres défauts : héparine de bas poids moléculaire, anticoagulation régionale au citrate en cas de très haut risque hémorragique, hirudine, argatroban, prostacycline, nafamostat mésilate, dont les places sont à préciser dans l’arsenal thérapeutique. L’objectif de cet article est de présenter les stratégies disponibles, en fonction de l’efficacité, de la sécurité et des modes d’utilisation des différents antithrombotiques pour l’épuration extrarénale continue.
During continuous renal replacement therapy, anticoagulation of the extracorporeal circuit is generally required for an optimal filter performance. Unfractionated heparin is still the most widely used anticoagulant. However, it presents some drawbacks: bleeding risk, heparin resistance, heparin-induced thrombocytopenia… Alternatives exist with their own disadvantages: low-molecular-weight heparin, regional citrate anticoagulation when bleeding risk is increased, recombinant hirudin, argatroban, prostacyclin, nafamostat mesilate for which indications need to be specified. The purpose of this article is to review the different strategies regarding efficacy, safety and implementation of anticoagulation during continuous renal replacement therapy.
