Le but de l’étude est d’évaluer l’utilisation d’un questionnaire semiquantitatif générique élaboré en CHU, utilisé par des professionnels de santé libéraux formés à la démarche, au début et à l’issue d’un parcours d’ETP proposé par un réseau de proximité en ville pour le diabète de type 2 (DT2) et l’obésité sans diabète (OB).Le questionnaire comporte des échelles analogiques visuelles remplies avec le patient et explorant plusieurs dimensions (cognitive, psychoaffective, motivationnelle) sous forme d’une cotation de 1 à 10, sauf pour le stade de Prochaska (SP) coté 1 à 5. Il est utilisé pour l’initiation (DEI) et l’évaluation (DEE) du parcours éducatif.Mille cent vingt DEI (OB : 49,7 %) et 275 DEE (délai moyen : 15,6 mois) ont été exploités. Les données sont insuffisamment renseignées (connaissances : 94,2 % ; psychoaffectif : 75 % ; motivation : 41,6 %, SP : 91,2 %). Les scores initiaux de connaissances (5,1 ± 2,0) ou de motivation (6,5 ± 1,6) ne sont pas différents entre patients OB et DT2. L’estime de soi et la confiance en soi sont plus faibles chez les OB (4,9 ± 2,2 vs 6,0 ± 2,1 ; 4,8 ± 2,2 vs 6,0 ± 2,0 ; p<0,0001), l’anxiété plus importante (6,2 ± 2,5 vs 5,4 ± 2,5 ; p<0,0001). L’évolution des scores est globalement favorable : connaissances (+1,5 ± 1,9, p<0,001), anxiété (– 0,4 ± 2,3, p=0,02), confiance en soi (+0,7 ± 2,0 ; p<0,0001), SP (+0,7 ± 1,1, p<0,0001). Le SP s’améliore chez 51,1 % mais diminue chez 16,7 % des sujets OB, s’améliore chez 60,6 % et diminue chez 9,5 % des sujets DT2.Bien qu’il soit encore difficile à utiliser, l’outil semble pertinent et opérationnel pour une utilisation en ville. Il permet de repérer des cibles prioritaires (parfois différentes entre DT2 et OB) tout en permettant le repérage des changements perçus par le soignant et le soigné.Les auteurs déclarent ne pas avoir d’intérêt direct ou indirect (financier ou en nature) avec un organisme privé, industriel ou commercial en relation avec le sujet présenté.
