In order to determine the intermediate term prognostic accuracy of EBCT as a screening test for coronary artery disease, we queried 1173 initially asymptomatic adults 3.6 years (range 38 to 47 months) after scanning. 1136 (97%) replied; efforts to contact the missing subjects remain under way.At the time of scanning, mean age was 53 +/- 11 years and median (25%, 75%) coronary calcium score (CCS) was 4 (0, 98). At follow-up, 40 patients had sustained cardiovascular death (2), nonfatal MI (14), CABG (14), or PTCA (10) [event rate 3.5%]. Median CCS was 655 (198, 972) among those with events and 4 (0, 91) among those without events (p < 0.0001)Sensitivity, specificity, positive and negative predictive values, overall accuracy (OA) and odds ratios are tabulated below for events, CCS thresholds, and groups of interest.In asymptomatic, apparently healthy adults, EBCT is highly predictive of cardiovascular events over the ensuing 3 to 4 years.
