L’éducation à la neurophysiologie de la douleur (NPD) est devenue un élément clé dans la prise en charge de la lombalgie chronique (LBC) [1,2]. Des outils pédagogiques ont récemment été développés tels que la brochure « PAIN fundamentals » [3] ou le site Internet www.retrainpain.org. L’objectif de cette étude consistait à examiner leur intérêt pour des patients LBC venant de bénéficier d’une séance d’éducation à la NPD et de comparer ces deux outils.Vingt-neuf patients LBC adressés à un centre de revalidation multidisciplinaire du rachis et venant de participer à une séance collective d’éducation à la NPD ont été inclus. Cinquante pour cent des patients ont été invités à consulter la brochure « PAIN fundamentals » puis, une semaine plus tard, le site Internet www.retrainpain.org; les autres patients ont bénéficié des outils dans l’ordre inverse. Une semaine après la réception de chaque outil, les participants étaient soumis à un questionnaire de satisfaction (QS) portant sur l’outil. Par ailleurs, le neurophysiology of pain questionnaire (NPQ-Fr) [4] était soumis avant la séance d’éducation, juste après celle-ci et en même temps que le QS.Les patients souffraient depuis en moyenne 9±12 ans. Un patient a indiqué ne pas avoir consulté le site et 4 n’ont pas utilisé la brochure. Quatre-vingt-cinq pour cent des autres patients ont rapporté avoir consulté les outils en entier. Le QS révèle, par ailleurs, que plus de 75 % des participants pensent que les outils constituent un complément utile à la séance, mais qu’ils ne peuvent pas la remplacer. Environ deux tiers estiment qu’ils devraient être distribués avant la séance et qu’ils les réutiliseront certainement/probablement dans le futur. Si la note attribuée aux deux outils est similaire (8/10), 60 % des sujets indiquent avoir une préférence pour le site. L’analyse de l’évolution des scores NPQ indique qu’ils augmentent de façon significative suite à la séance collective, mais qu’ils n’évoluent plus (p>0,05) par la suite et ce, quel que soit l’outil utilisé (Figure 1).Les deux outils étudiés ont été appréciés par la majorité des patients, avec une légère préférence pour le site Internet. Cette étude, qui confirme qu’une séance collective permet d’améliorer les connaissances relatives à la NPD, suggère par ailleurs, que ces outils ne permettent pas de les améliorer davantage. Ils semblent néanmoins constituer, d’après les patients, des outils complémentaires à la séance. Les résultats de cette étude mériteraient d’être confirmés sur un plus grand échantillon.
