l’ECMO permet une assistance circulatoire ainsi que l’oxygénation et l’épuration du CO 2 sanguin. Cette technique non dénuée de risques est grevée d’une morbimortalité propre. Nous décrivons ici les complications survenues dans notre service.étude observationnelle, rétrospective, monocentrique. Les patients de plus de 16 ans non atteints de cardiopathie congénitale ont été inclus sur une période de 3 ans allant de 2004 à 2007. Les résultats sont exprimés en médiane [valeurs extrêmes] ou moyenne ± déviation standard.Trente et un patients ont été inclus, leur âge était de 46 ± 17 ans, l’IGSII était de 57 ± 18. La durée de séjour en réanimation était de 13j [1-104]. Onze patients bénéficiaient d’une ECMO post chirurgie cardiaque, 8 d’une ECMO d’oxygénation (5 syndromes de détresse respiratoire aigue, 2 fibroses pulmonaires terminales et une ECMO après greffe pulmonaire) et 12 d’ECMO pour des états de choc dont 4 ECMO de sauvetage après arrêt cardiaque, 3 pour des embolies pulmonaires et 2 pour des intoxications médicamenteuses.Les ECMO, de marque MEDOS ® , étaient veino-artérielles fémoro-fémorales, une canule de réinjection était insérée dans 70 % des cas. La durée des ECMO était de 5,3 ± 5,7 jours. La décoagulation par de l’héparine visait à obtenir une héparinémie > 0,30 UI/ml.Une hémorragie significative définie par la nécessité de transfuser plus de 3 concentrés érythrocytaires survenait dans 50 % des cas. Elle concernait le site opératoire pour 9 patients. Sept patients présentaient une hémorragie au niveau des canules, on dénombrait un accident vasculaire cérébral hémorragique, 2 hémorragies intra-abdominales et une hématurie. Dans 9 cas, une réintervention chirurgicale était nécessaire. Les complications thrombotiques survenaient chez 13 patients dont : 6 ischémies aigues de membre inférieur, 4 thromboses des canules de reperfusion, une thrombose du circuit, une ischémie mésentérique, une thrombose de l’artère radiale et un accident vasculaire cérébral ischémique. Deux patients présentaient un sepsis au niveau du Scarpa ; enfin on note une décanulation accidentelle. Une hémolyse significative survenait 2 fois. Une thrombopénie inférieure à 100 G/l survenait chez 23 patients, et durait 7 ± 5,2 jours, se corrigeant toujours à l’arrêt de l’ECMO. La numération plaquettaire minimale était de 45 ± 20 G/l. Une insuffisance rénale aigue se développait chez 18 patients, dont 11 nécessitaient une dialyse. Une défaillance hépatique compliquait 11 ECMO. Parmi les complications on notait 5 pneumopathies acquises sous ventilation mécanique et une escarre talonnière.La mortalité en réanimation était de 65 %. La principale cause de décès était un état de choc (47 % des décès) ; 5 patients décédés avaient présenté un état végétatif persistant.les ECMO se compliquent fréquemment d’hémorragies, de thromboses, ainsi que de complications aspécifiques de décubitus. Ces évènements sont à mettre en balance avec le bénéfice vital attendu de ces techniques qui constituent souvent l’ultime recours thérapeutique.
