En cas de troubles trophiques des membres inférieurs chez le diabétique, une artériopathie est retrouvée dans 40 à 60 % des cas. Son évaluation conditionne le pronostic cicatriciel de la lésion et le taux d’amputation. Les modalités diagnostiques font appel notamment à la mesure des index de pression systolique et à l’échographie-doppler artériel suivant des recommandations spécifiques (ANAES 2002).Quarante-sept patients diabétiques présentant un trouble trophique podologique, hospitalisés dans notre service en 2006, ont bénéficié de 50 échographie-doppler. Chez tous nous avons évalué la concordance de l’examen avec les recommandations de l’ANAES et dans un sous-groupe de 15 patients, comparé les résultats avec les données de l’artériographie.La mesure des index de pression systolique (valeur moyenne 0,86) et l’analyse complète de l’arbre artériel sus-poplité ont été réalisées de façon systématique. La présence de médiacalcose était notée dans 15/50 (30 %) examens. La méthode de quantification des 13 sténoses retrouvées au niveau proximal n’est précisée que dans 3 examens. Concernant l’exploration des artères jambières, aucun examen n’a exploré les 3 axes de jambe sur toute leur longueur. La mesure des flux au niveau de la cheville a donc été réalisée de façon exclusive. La comparaison avec l’artériographie quant à la présence de sténoses significatives (> 50 %) montre une discordance de 30 % pour les lésions au niveau proximal. Au niveau distal, la sensibilité de l’échographie-doppler n’apparaît que de 70 %.Ces résultats montrent la médiocre performance de l’échographiedoppler dans notre population notamment pour le diagnostic des lésions jambières, la mesure des flux à la cheville ne permettant le dépistage que des lésions supérieures à 70 %. Or, ces lésions sont fréquentes chez les sujets diabétiques et l’efficacité de leur traitement bien démontré.La comparaison de cette série « historique » avec la série en cours à partir de 2007 devrait permettre de confirmer la pertinence des recommandations de l’ANAES pour le dépistage des lésions jambières dans le but de raccourcir le délai de cicatrisation des troubles trophiques des pieds des patients diabétiques.
