Right-sided diaphragmatic ruptures with herniation of intra-abdominal organs are very rare. It has never been reported before that hepatic torsion with porta hepatic and mostly the left hepatic lobe herniated in the right thorax. It also never has been reported that the antrum and some part of the body of stomach shifted to the right thorax, resulting the volvulus of the greater and the lesser curvature. We present a case of a 41-year old man who suffered a delayed presentation of an indirect traumatic diaphragmatic rupture. His symptoms were nonspecific. He was referred to our hospital for abnormal findings on chest X-rays. The definitive diagnosis was made through thoracoabdominal computed tomography.
