Objectif
Identifier les caractéristiques de l'enfant contact, du cas index ou de l'environnement, associées à l'infection ou à la tuberculose chez les enfants contacts dans une communauté urbaine en Indonésie.
Méthode
Les enfants qui étaient des contacts étroits d'un cas index de tuberculose pulmonaire ont été dépistés pour l'infection et la maladie à Yogyakarta, en Indonésie, d'août 2010 à décembre 2012. Les données du cas index et de l'enfant ont été recueillies de façon prospective et tous les enfants contacts ont subi une évaluation clinique, un test cutané à la tuberculine (TCT) et la radiographie pulmonaire. Ceux qui étaient cliniquement suspects de tuberculose ont également eu leurs crachats examinés par Xpert MTB/RIF et par la culture. Les enfants contacts ont été pris en charge conformément aux directives nationales, suivis pendant 12 mois et ont reçu une classification finale de soit tuberculose «maladie», soit infection tuberculeuse latente (ITL) ou “ seulement exposé”.
Résultats
269 enfants de 141 cas index ont été investigués. La classification finale était: tuberculose chez 25 (9%) et ITL chez 121 (45%). Le risque d'infection était significativement plus élevé si le cas index était de sexe féminin (AOR = 1,7; IC 95%: 1,0 à 2,8), avait une tuberculose à frottis positif du crachat (AOR = 3,0; IC95%: 1,5 à 6,0), ou dormait dans la même chambre (AOR = 1,7; IC95%: 1,0 à 2,9). Un résultat positif au TCT était indépendamment associé à un diagnostic de la tuberculose (AOR = 7,3; IC95%: 2,4 à 22).
Conclusion
Cette étude met en évidence le risque élevé et les facteurs de risque associés à la tuberculose et à l’ITL parmi les enfants contacts en Indonésie.
