Depuis 2005, la révision de la nomenclature, la nouvelle classification des anomalies du développement génitosexuel chez l’homme (DSD) et la découverte de nombreux gènes impliqués dans la détermination gonadique font partie des changements majeurs de l’approche de ces pathologies. En se basant sur ces avancées, cette revue est une mise au point sur la physiopathologie des DSD. Bien que l’étiologie des dysgénésies gonadiques commence par être expliquée grâce un screening des gènes impliqués, une majorité reste sans diagnostic moléculaire. En revanche, les 46,XY DSD par anomalie de la biosynthèse et du métabolisme de la testostérone sont confirmés par le séquençage des gènes responsables. De même, plus de 95 % des 46,XY DSD ayant un phénotype féminin complet et une fonction testiculaire normale sont dus à une mutation du gène du récepteur aux androgènes. En revanche, la majorité des autres 46,XY DSD ne sont pas des insensibilités partielles aux androgènes. D’énormes progrès permettent de mieux comprendre les pathologies liées au développement de l’ovaire et la physiopathologie des ovotestis. Les 46,XX DSD liés à une hyperandrogénie sont aussi abordés. Une approche multidisciplinaire pour un meilleur phénotypage et l’arrivée du séquençage du génome devraient améliorer notre compréhension des DSD. Néanmoins, l’intégration de toutes ces données par les cliniciens reste le futur challenge.
