Die offen-chirurgische Versorgung des sekundär frei in die Bauchhöhle rupturierten infrarenalen Aortenaneurysmas (AAA) weist eine sehr hohe Letalität auf: deutlich über 50%. Nur sehr wenige Patienten (etwa jeder 10.) werden lebend in die Klinik eingeliefert, und aufgrund der sehr schlechten Prognose, insbesondere für die älteren Risikopatienten, wird manchmal von einer offen-chirurgischen Operation gänzlich abgesehen (im eigenen Krankengut etwa 10%). Die zunehmende Erfahrung mit der endovaskulären Aortenrekonstruktion (EVAR) beim rupturierten AAA (rAAA) lässt eine Prognoseverbesserung mit deutlich niedrigerer Mortalität und Morbidität erwarten.
Bislang gilt hämodynamische Instabilität in vielen Zentren noch als Ausschlusskriterium für eine endovaskuläre Versorgung. Nach unseren Erfahrungen mit 31 Patienten im kompensierten hämorrhagischen Schock oder hämodynamisch stabil mit einer sog. „contained rupture“ berichten wir erstmalig über einen 73-jährigen Risikopatienten, der mit rAAA im dekompensierten hämorrhagischen Schock unter kontinuierlichen Reanimationsbedingungen erfolgreich durch EVAR therapiert wurde. Die sofortige und schnelle Blutungskontrolle in der hämodynamisch dekompensierten Situation wurde vor Implantation einer aorto-mono-iliakalen Endoprothese erreicht durch eine temporäre endoluminale Okklusion der suprarenalen Aorta mit einem 45-mm-Latex-Ballonkatheter.
Für die endovaskuläre Versorgung wurde ein endovaskuläres Notfallkit verwendet (Talent®, Medtronic AVE, Santa Rosa/CA, USA), bestehend aus einer aorto-mono-iliakalen Endoprothese, einer optionalen iliakalen Schenkelverlängerung und einem kontralateralen iliakalen Verschlussstopfen, dem sog. Endo-Okkluder. Zusätzlich wurde ein subkutan geführter femorofemoraler Cross-over-Bypass zur Revaskularisation des kontralateralen Beines angelegt. Das ausgeprägte abdominelle Kompartmentsyndrom (5 l intraabdominelles Blut) und ein persistierendes transfusionspflichtiges Endoleck Typ II über eine offene A. mesenterica inferior wurden nach Stabilisierung sekundär versorgt. Im 6-Monats-Follow-up war der Patient in einem exzellenten klinischen Zustand. Im Kontroll-CT wurden ein komplett exkludiertes AAA ohne Hinweis für Endoleckage und ein offener Crossover-Bypass festgestellt.
