Für die Diagnostik BCR-ABL-negativer myeloproliferativer Neoplasien (MPN) hat der Nachweis der JAK2(Januskinase 2)-Muta tion p.V617F und anderer Mutationen die Diagnosestellung erleichtert. Durch das immer bessere Verständnis von Signalwegen und epigenetischen Veränderungen bestehen Optionen, auch bei BCR-ABL-negativen MPN zielgerichtete und immunmodulatorische Therapien zu entwickeln. So kann bei Patienten mit Myelofibrose durch die Inhibition von JAK1 und JAK2 mit Ruxolitinib eine Verbesserung von konstitutionellen Symptomen und eine Verkleinerung der Milzgröße erreicht werden. Für die Diagnostik und Therapie der BCR-ABL-positiven MPN (chronische myeloische Leukämie, CML) konnten in den letzten Jahren erhebliche Fortschritte erzielt werden. Die zukünftigen Entwicklungen werden hier zeigen, ob unter bestimmten Bedingungen durch eine Medikation mit Tyrosinkinaseinhibitoren eine Heilung erzielt und die Therapie abgesetzt werden kann.
