Chronischer Schmerz ist als komplexes Syndrom zu verstehen, bei dem neben der Intensität, Qualität, Lokalisation und zeitlicher Charakteristika auch die subjektiven Beeinträchtigungen, Emotionen und Kognitionen von Bedeutung sind. Auf der Basis des biopsychosozialen Konzepts des Schmerzes muss auch die Therapie in multimodaler Weise die verschiedenen Dimensionen berücksichtigen.
Ein Baustein der multimodalen Schmerztherapie ist das Schmerzbewältigungstraining. Im Rahmen des Schmerzbewältigungstrainings wird angestrebt, dem Schmerzkranken eine zunehmende Kontrolle über den Schmerz zu ermöglichen, wobei als didaktische Grundregel das Prinzip der kleinen Schritte gilt. Neben der Vermittlung krankheitsspezifischer Informationen sollen die Patienten lernen, dysfunktionale Komponenten ihrer Schmerzkrankheit zu erkennen und diese zu verändern. Die Förderung der Gesundheit und Lebensfreude sowie eine Verminderung der psychophysischen Aktivierung durch Stressoren (Schmerz) sind Ziele dieses Trainings.
Das Programm der Schmerzbewältigung ist als ein Programm zur Selbsthilfe konzipiert. Schmerzbewältigungstraining ist indiziert bei Patienten, die an chronischen Schmerzen leiden oder Gefahr laufen, dass ihre Schmerzen chronisch werden.
