Die seltene Verletzungskombination aus lateraler Tibiakopffraktur und Abrissfraktur der Tuberositas tibiae erfordert ein Behandlungskonzept, insbesondere für die postoperative Nachbehandlung, das sich von den Vorgehensweisen bei der jeweiligen Einzelverletzung grundlegend unterscheidet. Wir berichten über einen Patienten mit diesem Verletzungsmuster, der im Rahmen eines berufsgenossenschaftlich versicherten Traumas durch passive Flexion im Kniegelenk mit gleichzeitiger aktiver Streckung des M. quadriceps femoris verunfallte. Es erfolgte eine multimodale osteosynthetische Versorgung und ein postoperatives Behandlungskonzept aus Ruhigstellung in einer Oberschenkelschiene für 6 Wochen, isometrischer Krankengymnastik und Bewegungsverbot im Knie für 4 Wochen. Dies ist eine effektive und erfolgversprechende Versorgung bei einer Verletzungskombination, für die bisher kein Verfahren etabliert ist.
