We report two cases of patients with chronic respiratory disease who experienced an asphyxia after aspirating a sucralfate tablet that occluded a lobar bronchus. In adults, a foreign body is a rare cause of acute respiratory failure from tracheobronchial occlusion. The sucralfate tablet has the physical property of expanding rapidly when wet (contact with mucosa). After aspiration, the tablet expands to a larger size and can occlude a lobar bronchus, causing acute respiratory failure. In patients at risk of aspiration, we recommend the use of sucralfate in liquid or suspension form.Nous rapportons deux cas d'asphyxie aigue par obstruction d'une bronche lobaire, apres inhalation d'un comprime de sucralfate par des patients insuffisants respiratoires chroniques. Chez l'adulte, l'inhalation d'un corps etranger est rarement responsable d'une insuffisance respiratoire aigue par obstruction tracheobronchique. Le comprime de sucralfate, par sa propriete d'expansion volumique au contact d'une surface humide, telle que celle d'une muqueuse, peut apres inhalation, obstruer une bronche de gros calibre et etre a l'origine d'une insuffisance respiratoire aigue. Chez les patients a risque de fausses routes, l'utilisation d'une autre forme galenique (granules pour suspension buvable) doit etre recommandee.
