Le but de ce travail était d'évaluer l'intérêt du Cyfra 21-1 comme indicateur de l'efficacité thérapeutique et comme facteur pronostic dans le cancer du poumon. Nous avons retenu 42 patients atteints d'un cancer primitif du poumon sur le critère d'un taux sérique de Cyfra 21-1 élevé (> 3,3 μg/L) au moment du diagnostic. Chaque cas a été défini histologiquement: 17 cancers épidermoïdes, 11 adénocarcinomes, 8 cancers à grandes cellules et 6 cancers à petites cellules. Des dosages itératifs de Cyfra 21-1, d'ACE et de NSE ont été effectués dès la mise en place de la première thérapeutique (chimiothérapie n = 36, radiothérapie n = 3, chirurgie n = 3). Les valeurs ont été analysées en fonction de la réponse au traitement et de la survie. Chez 35 des 42 patients, on constate une décroissance significative (DS) du taux de Cyfra 21-1 (diminution de plus de 50 % et/ou retour à la normale); parmi eux, 27 reviennent à la normale (RN). Vingt-deux des patients avec DS et 18 des patients avec RN ont une réponse clinique faisant du Cyfra 21-1 un indicateur modéré en terme de valeur prédictive positive. En revanche, la valeur prédictive négative de 100 % est très intéressante puisqu'elle signifie qu'une réponse clinique est toujours associée à une DS du Cyfra 21-1. Onze rechutes cliniques ont été observées, correspondant à une augmentation du Cyfra 21-1 dans huit cas (72%). Trois de ces élévations ont précédé d'un mois les signes cliniques et/ou radiologiques. L'étude des courbes de survie montre une différence significative entre les patients chez lesquels on a observé une DS et/ou une RN et les autres.
We have investigated the usefulness of Cyfra 21-1 as indicator of therapy effectiveness and prognosis in lung cancer. Forty-two patients with primary lung cancer were selected on the basis of a high Cyfra 21-1 serum level (> 3,3 μg/L) at diagnosis time. All cases were histologically proved: 6 small cell carcinomas, 17 squamous cell carcinomas, 11 adenocarcinomas and 8 large cell carcinomas. Serial monitoring of Cyfra 21-1 were performed during modalities of first-therapy (chemotherapy 36, radiotherapy 3, surgery 3 cases). The serial values were analysed according response to treatment and overall survival. Thirty-five of 42 patients had a significant decrease (SDS) (> 50% or return to normal) of Cyfra 21-1 serum level and 27 returned to normal values (NV). Twenty-two (62%) of SDS patients, and 18 of NV patients (66%) had a clinical response to therapy, making Cyfra 21-1 a moderate indicator in terms of positive predictive value. Clinical response, however, was always associated with a SDS of Cyfra 21-1. Eleven clinical relapses were observed, that were reflected by a positive Cyfra 21-1 in eight (72%) cases at the time of radiological or clinical relapse. In three patients, Cyfra 21-1 values preceded the clinical detection by one month. Survival data was available for 36 patients. A statistically significant difference (p < 0,05) was found in survival depending on return to normal of Cyfra 21-1 (or SDS) during therapy or not.
