La place de la médecine dans le football professionnel est trop souvent réduite au suivi traumatologique. La notion d'état de forme relève très souvent de données empiriques voires subjectives de la part des entraîneurs ou des préparateurs physiques. Lorsque la fatigue s'installe, il est déjà trop tard, et parfois cela bascule vers un vrai syndrome de surentraînement souvent dangereux pour la santé du sportif et souvent fatal pour sa carrière.Le suivi biologique médical longitudinal mis en place par le ministère des Sports ne peut malheureusement être le reflet précis de l'état de forme tout au long d'une saison car d'une part il reste ponctuel à raison de trois prélèvements par an, et d'autre part, il ne fait appel qu'à des marqueurs biologiques, qui ne sont, pas toujours spécifiques de la fatigue.De nombreux travaux ont montré que l'augmentation de la charge de travail physique peut aboutir à de la fatigue qui se caractérise par une diminution des performances physiques et mentales. L'état de fatigue a pour origine un déséquilibre de la balance énergétique entre les apports nutritionnels et la dépense liée au travail musculaire. Des contraintes supplémentaires telles que des perturbations du rythme veille-sommeil ou le stress psychologique facilitent cet état. Ces différents facteurs agissent ensemble pour modifier le statut hormonal et immunitaire.Le but de notre étude a été de déterminer dans le cadre du suivi biologique de l'athlète de haut niveau des marqueurs spécifiques de l'apparition d'un état de fatigue voire de surentraînement. Nous avons donc réalisé cette exploration sur les joueurs de football professionnel du Paris Saint-Germain durant trois années consécutives depuis la saison 2001–2002.Les principaux résultats mettent en évidence qu'une saison sportive chez le footballeur entraîne une modification des réponses hormonales des axes corticotrope et gonadique. Le rapport éstostérone/cortisol permet d'appréhender s'il existe ou non une adaptation à l'entraînement et ponctuellement dans le cadre d'un match. L'analyse du CFF (outil non invasif) permet d'évaluer l'état de vigilance et un éventuel niveau de fatigue centrale.
The scope of medicine in professional soccer is too often reduced to a traumatologic follow-up. The concept of fitness is often based to subjective parameters determined by coaches or physical preparer. When fatigue occurs, it is already too late, and sometimes the overtraining syndrom appears inducing dangerous consequences for the health of the athlete and fatal for his career.The longitudinal follow-up established by Sport Minister cannot unfortunately be the specific reflect of the state of form of a sportman during the course of the sporting season because it made only three time in the sporting season and the biological markers are not specific to the state of tiredness. Numerous works have shown that enhanced physical load could result in fatigue which is characterised by a decrease in physical and mental performance. Fatigue is caused by an energy imbalance between nutritional intake and energy expenditure due to muscular work.Supplementary chronobiological and psychological stress facilitates fatigue. The general effect of these different factors is responsible for hormonal and immune changes.The aim of our study was to determine within the scope of the longitudinal follow-up specific markers to the appearance of state of tiredness or overtraining. Thus, we have realized this experimentation on the professionnal soccer players of Paris Saint Germain Football Club for three seasons running.The main results show that a sporting season of a soccer player induces modifications of hypothalamo-pituitary and gonadal axis. The ratio testosterone/cortisol allows to appreciate the adaptation of the training load and during league match. The critical flicker frequency (CFF) analysis allows to evaluate vigilance state and central fatigue.
