L’obésité est caractérisée par un état inflammatoire chronique, se traduisant par l’augmentation des taux plasmatiques de facteurs inflammatoires tels que les cytokines, les chimiokines et les microARNs (miR). Tous ces marqueurs contribuent au développement des complications métaboliques telles que le diabète de type 2 et la résistance à l’insuline. Nous avons mené une étude clinique afin d’identifier les miR dont l’expression est modulée en fonction du degré d’obésité.La population étudiée comportait 101 sujets obèses non diabétiques présentant les caractéristiques suivantes (moyenne±SD) : âge 40,8±12,5ans ; IMC 40,5±5,8kg/m2 ; poids 112,7±18,9kg ; tour de taille 115,9±10,3cm. Parmi cette cohorte, nous avons choisi huit patients contrastés sur l’IMC, quatre étaient obèses (IMC 32,12±0,35kg/m2)et quatre obèses morbides (IMC 46,55±1,6kg/m2). Notre étude a été approuvée par le comité d’éthique local et tous les participants ont donné un consentement éclairé écrit. Tous les patients ont bénéficié d’un bilan nutritionnel et métabolique comprenant la glycémie et l’insulinémie à jeun, les paramètres lipidiques, le dosage de certains marqueurs inflammatoires tels que les adipokines (adiponectine, leptine), les cytokines (TNFα, IL1, IL6), les chimiokines (CXCL1, CXCL8, CXCL10, CCL2, CCL19) et les miR (miR-155, miR-146a, miR-150, miR-302a, miR-124, miR-320a). Des miRNA PCR arrays ont été réalisés sur les échantillons des huit patients afin de quantifier un certain nombre de miR. Des qPCR ont été également réalisés sur les autres échantillons.L’analyse des miRNA PCR arrays à partir des échantillons de la sous cohorte (n=8) a permis de mettre en évidence une expression différentielle de 3 miR (miR-150, miR-124 et miR-302a) au niveau plasmatique entre les obèses et les obèses morbides. Nous avons également mis en évidence une augmentation de certaines chimiokines chez les obèses morbides (CCL2, CXCL10 ; p<0,05). Dans la cohorte totale (n=101), nous avons montré que ces miR régulés étaient corrélés avec les paramètres de l’obésité (poids, IMC, tour de taille). En analyse univariée, l’expression du miR-150 était inversement corrélée avec le taux du cholestérol total (r=–0,29 ; p<0,05), l’expression du TNFα (r=–0,31 ; p<0,01) et d’IL6 (r=–0,32 ; p<0,001). Le miR-302a était inversement corrélé avec le poids (r=–0,22 ; p<0,05), l’IMC(r=–0,27 ; p<0,01), les taux de protéine C réactive (CRP ; r=–0,31 ; p<0,01) et de leptine (r=–0,26 ; p<0,05). Le miR-124a était également inversement corrélé avec le poids (r=–0,2 ; p<0,05), l’IMC (r=–0,3 ; p<0,01),les taux de CRP(r=–0,28 ; p<0,01) et de leptine (r=–0,41 ; p<0,01). En régression linéaire multivariée, la relation inverse entre l’expression des miR-124 et miR-302a et le poids persistait après ajustement pour le sexe et l’âge (p<0,05). En outre, la relation inverse entre l’expression du miR-124 et le taux de leptine persistait après ajustement pour le sexe, l’âge et l’IMC (p<0,05). Cette même relation était perdue pour le miR-302a.Les miR pourraient être utilisés comme des biomarqueurs ou des cibles thérapeutiques dans le traitement de l’obésité et de ses complications.
