The STT joint was examined in 73 fresh cadaveric specimens (25 male and 48 female with an average age of 84 years) with a view to study the incidence and characteristics of degenerative changes in this joint. The articular degeneration was graded from 0 to 3 according to increasing loss of cartilage and the location of the changes was noted. At the same time, the presence and extent of concomitant trapezio-metacarpal arthritis was noted. 61 of the 73 hands (83.3%) were found to present STT arthritis. Degeneration of the trapezoid articular surface was : — more frequent : 53 hands (72.6%) as compared to 48 (65.7%), — of greater severity : 40 cases (55%) of grades 2/3 as compared to 25 (34%), than that of the trapezium. Concomitant or isolated arthritis in the trapezometacarpal joint (90.4%) was present in 66 of the 73 hands (79%) examined with grade 3 changes in 13 cases. The apparent predominance of degeneration in the scapho trapezoidal articulation could, perhaps, lead us to assume that this might be the site of origin of STT arthritis. It could also explain the persistence of symptoms following prosthetic replacement of the trapezium.
L'articulation STT a été examinée sur 73 spécimens de cadavres frais (25 hommes et 48 femmes d'un âge moyen de 84 ans), afin d'étudier l'incidence et les caractéristiques des modifications dégénératives de cette articulation. La dégénérescence articulaire a été notée de 0 à 3 selon la perte croissante de cartilage, et la position des modifications a été relevée, ainsi que la présence et l'état d'avancement de l'arthrose trapézo-métacarpienne associée. La présence de l'arthrose STT a été observée dans 61 des 73 mains (83,3%). La dégénérescence de la surface articulaire du trapézoïde était : — plus fréquente : 53 mains (72,6%), par rapport à 48 (65,7%); — plus grave : 40 cas (55%) des niveaux 2/3, comparée à 25 (34%) que celle du trapèze. L'arthrose associée ou isolée de l'articulation trapézo-métacarpienne (90,4%) était présente dans 66 des 73 mains (79%) examinées, avec des modifications de niveau 3 dans 13 cas. La prédominance apparente de dégénérescence de l'articulation scapho-trapézoïdienne pourrait, peutêtre, nous amener à penser que ce pourrait être là le site originel de l'arthrose STT. La presencia de poliartritis STT se observó en 61 de las 73 manos (83,3%). La degeneración de la superficie articular del trapezoide era : — más frecuente : 53 manos (72,6%), con referencia a 48 (65,7%). — más grave : 40 casos (55%) de los niveles 2 y 3, comparado a 25 (34%) que la del trapecio. La poliartritis asociada o aislada de la articulación trapezo metacarpiana (90,4 %) se hallaba presente en 66 de las 73 manos (79%) examinadas, con modificaciones de nivel 3 en 13 casos. El predominio aparente de la degeneración de la articulación escafo trapezoidea podría, tal vez, llevarnos a pensar que este podría ser el sitio original de la poliartritis STT. Esto podría igualmente axplicar la persistencia de síntomas a pesar del remplazo del trapecio por una prótesis.
