L’implication des macromycètes en pathologie humaine a été probablement sous-estimée. Ces champignons dits émergents sont de plus en plus décrits dans les processus infectieux, notamment grâce à l’amélioration des techniques d’identification. L’exemple type est la sinusite à Schizophyllum commune. Nous décrivons ici l’implication du macromycète Trametes polyzona dans deux processus infectieux pulmonaires survenus à l’île de la Réunion en 2014.L’identification mycologique morphologique n’était pas possible, en raison d’absence de sporulation de la culture. L’identification définitive a été réalisée par séquençage de l’ADN ribosomal fongique (ITS, et 28s).Les deux cas sont survenus sur des terrains débilités.Le premier cas est survenu chez un homme de 50 ans, aux antécédents de bronchite chronique, et d’exogènose, pris en charge pour une pneumopathie fébrile hypoxémiante. Le scanner a mis en évidence un abcès du lobe supérieur gauche, drainé chirurgicalement par lobectomie. L’infection pulmonaire fongique par T. polyzona a été mise en évidence de façon certaine par la culture de la pièce opératoire et par l’anatomopathologie authentifiant des filaments mycéliens, suggérant l’implication directe du champignon dans le processus infectieux. L’infection a été traitée avec succès par Amphotéricine B liposomale pendant 21 jours.Le deuxième cas est survenu chez un enfant de 2 ans, sur un terrain d’insuffisance respiratoire chronique (protéinose alvéolaire). La radiographie standard a mis en évidence une opacité du lobe inférieur droit. La culture du LBA était positive à T. polyzona. Le patient a été traité par Amphotéricine B liposomale pendant 40 jours, et est décédé dans les suites d’une défaillance multiviscérale.T. poly zona, est un basidiomycète filamenteux, appartenant a l’ordre des polyporales prévalent en zone tropicale. À notre connaissance, ce macromycète n’a jamais été rap porte dans un processus infectieux, et pourrait être considère comme un pathogène opportuniste en milieu tropical.
