Les stades avancés de la maladie de Kienböck sont traités par différentes techniques, dont la technique de Graner, presque oublié actuellement.Nous présentons les résultats à long terme d’une série de 4 interventions de Graner.Nous avons revus 4 patients avec un recul de 25ans, 2 femmes et 2 hommes âgés en moyenne de 37ans au moment de l’intervention. Deux étaient travailleurs manuels. Dans 3 cas, l’intervention de Graner a été réalisée en première intention et dans 1 cas, après échec d’accourcissement du radius au stade IIIa.Trois patients ont consolidé, la 4 e a été reprise par arthrodèse radiocarpienne. Le score de DASH était en moyenne de 36,6, la douleur était de 3,25/10, l’arc de mobilité était d’environ la moitié de celui du côté controlatéral, la force de la poigne était de 80,9 % du côté controlatéral. Dans les 3 cas consolidés, on notait un remodelage des surfaces articulaires radiocarpiennes.L’intervention de Graner est logique, parce qu’elle a pour but de créer une néo-arche condylienne carpienne, soit en fusionnant le lunatum avec le capitatum (Graner I), soit en remplaçant le lunatum par la tête du capitatum (Graner II et III). Cette intervention ancienne ne doit plus faire partie de l’arsenal thérapeutique de la maladie de Kienböck, car elle possède des inconvénients rédhibitoires : risques de nécrose et de pseudarthrose de la tête du capitatum, arthrose à long terme, prélèvement d’un greffon autologue spongieux.
