Bien que les immunoglobulines intraveineuses (IgIV) soient utilisées depuis une vingtaine d’années dans les maladies auto-immunes et inflammatoires, les mécanismes moléculaires à la base de leur effet bénéfique ne sont pas complètement élucidés. Ces mécanismes dépendent des fragments constants (Fc) et/ou variables (Fab’)2. Les IgIV interagissent avec de nombreux composants du système immunitaire comme les récepteurs Fc, le complément, les cytokines, les lymphocytes T et B, les cellules dendritiques, les granulocytes et les cellules NK. Nous présenterons dans cette revue les données récentes sur les mécanismes d’action anti-inflammatoire des IgIV, en mettant l’accent sur l’importance du fragment Fc sialylé. Ces avancées permettront peut-être de concevoir de meilleures stratégies thérapeutiques contre les maladies auto-immunes et inflammatoires.
Despite their widespread use since many years in autoimmune and inflammatory disorders, the mechanisms of action of IVIg have not been completely understood. These mechanisms depend on Fc and/or F(ab’)2. IVIg interacts with the different components of the immune system: Fc receptors, complement, cytokines, T and B lymphocytes, dendritic cells, granulocytes and NK cells. Here, we discuss the recent advances in the understanding of the mechanisms of action of IVIg, in particular the importance of the sialylated Fc fragment. These advances maybe help us conceive better therapeutic strategies against autoimmune and inflammatory disorders.
