Les diabètes atypiques représentent 1 à 2 % des diabètes. Néanmoins la distinction entre un diabète de type 1 ou 2 avec les autres formes peut être difficile. Une approche clinique associée à une exploration biologique adaptée oriente le diagnostic génétique. Notre objectif est d'évaluer la prévalence et relever l'intérêt du dosage des anticorps anti GAD.Il s'agit d'une étude prospective, menée du 1er janvier 2015 au 1er septembre 2015 dans la Région des Grands-Lacs. En faveur du diabète monogénique (MODY, mitochondrial) et syndrome de Wolfram, nous avons recherché les antécédents personnels d'atteinte rénale, oculaire ou auditive et la forte hérédité diabétique associant ou non des complications microangiopathiques. En faveur du diabète de l'africain et de type LADA, nous avons recherché le début cétonurique qui ayant évolué ou non vers des hypoglycémies, avec une sensibilité particulière aux antidiabétiques oraux, notamment les sulfamides. Les anticorps anti GAD ont été demandés en cas de suspicion de MODY3, diabète LADA ou diabète cétonurique de l'africain.Nous avons suspecté 707 cas de diabète atypique sur les 1 985 diabétiques hospitalisés (35,6 %) durant la période d'étude, avec une prédominance de type LADA, dont 38 cas ont été confirmés par la positivité des anticorps anti GAD. Le diabète mitochondrial a été évoqué chez 13 patients, le syndrome de Wolfram dans 8 cas. Le diabète cétonurique de l'africain a été confirmé chez 5 patients et diabète MODY 3 suspecté chez 3 patients devant des anticorps anti GAD négatifs et un âge de moins de 25 ans.La prévalence des diabètes atypiques dans notre région est estimée à 35,6 % supérieure à celle décrite au Maroc (16,9 %). Il est important de savoir orienter le diagnostic en présence d'arguments cliniques présomptifs. Les anticorps anti GAD représentent un bon outil diagnostique devant la difficulté de réaliser des études génétiques et devra être disponible dans notre Région. L'immunothérapie avec ENTANERCEPT (= ENBREL®) devrait être une alternative.
