L’insulinorésistance est une composante majeure du diabète de type 2 et se caractérise notamment par des altérations du métabolisme hépatique du glucose. En effet, chez des rongeurs obèses, l’insulinorésistance entraîne une activation de la néoglucogénèse hépatique contribuant ainsi à l’hyperglycémie chronique de ces animaux. Récemment, l’activation de la voie du stress du réticulum endoplasmique (ER stress) a été décrite dans le foie des rongeurs obèses insulino-résistants (souris ob/ob souris sous régime gras). In vitro, il a été montré que l’activation de la voie du ER stress inhibait la voie de signalisation insulinique dans les hépatocytes. Nous avons émis l’hypothèse que l’activation de la voie ER stress pourrait jouer un rôle important dans l’insulinorésistance hépatique observée chez les rongeurs obèses.Dans ce travail, nous avons inhibé la voie du stress du réticulum endoplasmique in vivo dans le foie de la souris ob/ob en surexprimant la protéine chaperone BiP/GRP78 à l’aide d’un adénovirus. Nous avons analysé les conséquences de cette inhibition sur la voie de signalisation insulinique et la sensibilité à l’insuline de ces animaux.Nous retrouvons une forte induction des marqueurs du stress du réticulum endoplasmique dans le foie des souris ob/ob par rapport aux souris ob/+. La surexpression hépatique de la protéine BiP/GRP78 chez ces animaux entraîne une forte diminution de ces marqueurs. L’inhibition de la voie ER stress s’accompagne d’une restauration de la voie de signalisation insulinique PI3 kinase dépendante ainsi que d’une amélioration des paramètres métaboliques de ces animaux. Nous avons évalué la sensibilité à l’insuline en utilisant la technique du clamp euglycémique hyperinsulinémique couplée à la perfusion de glucose tritié. Les souris ob/ob surexprimant BiP/GRP78 dans le foie présentent une augmentation de la sensibilité hépatique à l’insuline sans modification majeure de la sensibilité périphérique.Nos résultats montrent l’implication de la voie du stress du réticulum endoplasmique dans l’insulinorésistance hépatique observée chez la souris ob/ob. Une inhibition pharmacologique de la voie ER stress pourrait donc être un moyen efficace pour améliorer la sensibilité à l’insuline dans des modèles d’insulinorésistance.
