Alcool et xenobiotiques partagent la meme voie d'oxydation microsomale , principalement localisee dans le reticulum endoplasmique des cellules hepatiques. Cette voie fait intervenir des enzymes appartenant a la super-famille des cytochromes P450 et permet d'expliquer un certain nombre d'interactions d'ordre pharmacocinetique ou toxique entre alcool et xenobiotiques. L'enzyme cle de la voie microsomale d'oxydation de l'ethanol est le cytochrome P450 2E1 (CYP2E1). Il est induit par la consommation chronique d'alcool, son activite au niveau du foie etant multipliee par trois a cinq chez les sujets alcooliques. Cette induction, qui associe egalement a un moindre degre les cytochromes P450 CYP3A4 et 1A2, contribue a la tolerance metabolique qui se developpe chez l'alcoolique et qui concerne non seulement l'alcool mais aussi certains medicaments. Cette tolerance metabolique persiste plusieurs jours apres arret de la consommation d'alcool. Par ailleurs, le CYP2E1 a une grande capacite a activer de nombreux xenobiotiques en metabolites toxiques ou carcinogenes. Cette bioactivation concerne des medicaments utilises couramment tels que le paracetamol, des anesthesiques (enflurane, halothane), des solvants industriels (le benzene ou ses derives), des solvants halogenes (tetrachlorure de carbone, trichlorethylene) et des nitrosamines presentes dans l'alimentation ou la fumee de cigarette. Ainsi, la consommation excessive d'alcool, qui a pour effet d'induire le CYP2E1, augmente la susceptibilite des individus aux effets toxiques ou carcinogenes de ces xenobiotiques.

Drugs currently used such as paracetamol, anesthetics (enflurane, halothane), industrial solvents (benzene or its derivatives), halogenated solvents (CCl 4 , trichlorethylene) and nitrosamines which are present in food or tobacco smoke are included. Therefore, heavy consumption of alcohol, which results in CYP2E1 induction, increases individual susceptibility to the toxic or carcinogenic effects of these xenobiotics.
