Le diagnostic de valves de l’urètre postérieur (VUP) reste assez fréquent même si la prise en charge anténatale en a modifié l’évolution par le dépistage des formes sévères. Nous décrivons les données anténatales, néonatales et le devenir des enfants porteurs de VUP pris en charge entre 1997 et 2007.26 enfants ont eu un diagnostic de VUP, soit environ 1 cas pour 13 000 enfants nés vivants.en anténatal, dans 21 cas, un diagnostic (Dg) de dilatation bilatérale des voies urinaires (VU) a été porté. 14 sont nés dans une structure hospitalière adaptée. Malgré ce Dg anténatal 4 enfants ont été pris en charge entre J7 et J30, devant un tableau d’infection urinaire (IU). Dans 1 cas, un Dg de polykystose rénale avait été évoqué. Dans 4 cas, le Dg n’avait pas été évoqué et ces enfants avaient en fait une forme mineure. Le Dg de VUP a donc été fait après 1 mois de vie chez 6 enfants. La prise en charge a consisté en la mise en place d’un KT sus-pubien avec résection secondaire des VUP. La cystographie montrait un reflux vésico-uretéral chez 16 enfants, bilatéral chez 11. 8 cas présentaient une insuffisance rénale (IR) néonatale (créatininémie > 100 mcmol/l, après drainage vésical) et 3 d’entre eux avaient une IU sévère inaugurale. Devant une aggravation de l’IR malgré le drainage vésical, 3 ont eu une urétérostomie bilatérale avec amélioration de la fonction rénale (FR). Une néphrostomie percutanée transitoire a été nécessaire dans un cas.parmi ces 26 enfants, 18 ont maintenant plus de 2 ans don’t 2 sont perdus de vue. 13 ont une FR normale, 2 ont une IR modérée (créat 65 mcmol/l) et un seul a évolué vers l’IRT et a bénéficié d’une transplantation rénale préemptive à 7 ans. 5 présentent des troubles mictionnels après l’acquisition de la propreté, d’évolution favorable après éducation mictionnelle et/ou posthectomie.l’affirmation du diagnostic de VUP est parfois difficile en anténatal et la prise en charge post-natale immédiate n’est pas toujours adaptée : les IU graves révélatrices sont fréquentes dans notre série. A plus long terme, dans les conditions actuelles de la prise en charge, en particulier en anténatal, le pronostic fonctionnel rénal semble relativement satisfaisant.
